Bluegrass Care Navigators connects patients with caregivers who have exactly the right expertise to meet their health care needs and help handle the activities of everyday living. Our team provides unsurpassed private duty nursingpalliative carehospice care and more.

Position Qualifications:

  • Graduate of an accredited seminary with a Masters of Divinity degree or a similar master’s level degree which is acceptable for certification with a professional chaplains organization (APC, NAJC, or NACC). Ordination or equivalent endorsement and in good standing in his/her faith group. Two units of Clinical Pastoral Education.
  • Knowledge of the process of death, dying and bereavement.
  • Knowledge and a basic understanding of the interpersonal dynamics involved in working with an interdisciplinary healthcare team. Ability to give and receive feedback.
  • Knowledge and a basic understanding of spiritual issues of terminally ill patients and their families.
  • Knowledge of differing faith traditions and the ability to work with patients’ families from various faith traditions.
  • Skilled public speaker and good community organizational skills.
  • Current driver’s license and auto liability coverage
  • CPR certification, negative TB skin test, pre-employment physician’s health clearance

Summary of Job Responsibilities:

The Hospice Chaplain is responsible for the provision of spiritual care services to patients/families/caregivers of Hospice of the Bluegrass, either directly or through coordination of care with other spiritual counselors. The hospice chaplain shares the responsibility for other aspects of patient care except where limited by licensure.

Bluegrass Care Navigators (formerly Hospice of the Bluegrass) is a nationally respected hospice, having cared for thousands of families in our Kentucky communities since 1978, providing hospice services to approximately 750 patients daily in 32 counties located in central, northern and eastern Kentucky. Our services currently include:

  • Bluegrass Extra Care – a private pay service offering personalized home care
  • Bluegrass Transitional Care – help with hospital to home
  • Bluegrass Palliative Care – pain and symptom relief
  • Bluegrass Hospice Care – expert care for the final months of life
  • Bluegrass Grief Care – support through anticipatory grief and bereavement
